Dental (Grades K, 3, &7)

In accordance with Pennsylvania state regulations, all children upon original entry to school, shall be given a dental examination by a dentist.  Please supply a copy of this report to the school nurse.  You may get the form from the school nurse or by .

Medical Examination (Grades K, 6, & 11).

School law provides for a medical examination of every child upon entry into school. The District will accept reports of conducted physical exams completed within one (1) year prior to the student’s entry into the grade in which the exam is required.  Please supply a copy of this report to the school nurse.  You may get the form from the school nurse or by downloading it below.

The only exemptions to the school laws for  immunizations are:

Medical reasons;
Religious beliefs; or
Philosophical/strong moral or ethical conviction.

If your child is exempt from immunizations, he or she may be removed from school during an outbreak.
