Dependents can be on health care coverage until age 26. Coverage will last until the last day of the month that they turn 26. There is NOT a requirement to be a full time student to remain on this benefit.

- Dependents can be on vision coverage until age 19 or age 23 if they remain a full time student. You should receive dependent certification forms directly from the vision provider, to confirm dependents are full time students or click here: PSEA Dependent Certification Form (PSEA accepts the student’s full time schedule printed from their portal as an official enrollment verification form). If they are not full time students or you don't complete the form they will be removed from vision coverage. The Vision provider PSEA will be sending out information for COBRA.

If they are not full time students or you don't complete the form they will be removed from dental coverage then will have the option of continuing this benefit through COBRA (will have to pay to continue this benefit.) we work with a third party vendor called WageWorks.

Workers Compensation
If you are injured on the job please review and complete the following documents:
- Report of injury - This form captures the data needed when reporting a claim.
- Panel with Employee's Rights and Duties - You must select from one of the health care providers on the attached list.
- Medical Authorization - This is to be given at the time of the injury.
- First Fill Form - This can be presented to the pharmacy at the time you are having your first prescription filled. The pharmacy will then direct bill WorkPartners for the prescription.
In order to receive your Workers Compensation claim number, send finished packet to HR via email to the attention of the following or fax to 717-530-2811
